Fill out the Initial Report to Superintendent for your student's first year of homeschooling.
Submit your student's Vaccination Records with the initial report, when a new child is added, and again when any child reaches age 12. Alternatively, a NOTARIZED Conscientious Exemption may be submitted. (Parents/guardians must not sign until directed to by a notary public, available at most banks.)
Use the Letter of Intent to Continue Homeschooling after your student's first year of homeschooling.

Remember to keep copies of everything you submit!
Homeschoolers must keep records of the work their child is doing in the following subjects:
· basic communication skills including reading, writing, literature, and fine arts
· mathematics and science
· social studies including history, geography, economics, government, and citizenship
· health and physical education
There is no specific method required. Use whatever is easy for your family—a planner, spreadsheet, web diagram, lists and receipts of library books, photographs, blog, a social media account, etc. Records should include class schedules, copies of materials used for instruction, and descriptions of methods used to assess student achievement.
When your child is high school age, you will need to create a transcript of course work.
